DataClarity Corporation offers coverage from United Healthcare. With United Healthcare, you can see in-network doctors for just a copayment.

You can be confident knowing that your plan offers:

  • Provider directory to help find a physician or facility in your area or while you’re traveling
  • – one-stop site for locating a participating retail pharmacy by ZIP code, looking up possible lower-cost medication alternatives, and comparing medication pricing and options.

With United Healthcare coverage, you may visit any provider in the United Healthcare network, and you’ll be covered by your plan. You may also visit out-of-network providers, but your coverage level will be different. Log in to the Member Services section of for complete details.

  • Office visits – In-network primary care doctor or specialist visits are covered by a copayment. Our current plan also includes coverage of annual preventive care.
  • Inpatient and Outpatient Hospital Services – When you’re hospitalized or when you have outpatient procedures performed, you’ll typically pay a deductible and coinsurance.
  • Prescription drug coverage – You have access to United Healthcare’s large network of pharmacies. Prescription drugs are covered based on different classifications, depending on the type of drug you’re prescribed. Visit the “Find a Doctor” section of for complete details.

DataClarity Corporation offers dental coverage through MetLife. In addition to offering an extensive national network and a sophisticated claim system, MetLife also ensures proper credentialing and practice patterns of participating dentists.

MetLife’s Preferred Dentist Program (PDP) network allows employees to select the dentist of their choice – an “in-network” dentist or an “out-of-network” dentist. PDP discounts typically range from 15-45%.

Additional dental services available include:

  • Oral Health Library, a one-stop shop for dental information
  • Dental Procedure Fee Tool available on MetLife’s member website to calculate potential costs prior to exams
  • Provider Directory to help find a dentist or orthodontist
  • One of the nation’s largest dental networks with more than 158,000 participating PDP dentist access points nationwide, including 37,000 specialists

DataClarity Corporation offers vision coverage from MetLife. With your Vision Preferred Provider Organization Plan, you can:

  • Go to any licensed vision specialist and receive coverage. Just remember your benefit dollars go further when you stay in network.
  • Choose from a large network of ophthalmologists, optometrists and opticians, from private practices to retailers like Costco® Optical and Vision works.
  • Take advantage of our service agreement with Walmart and Sam’s Club—they check your eligibility and process claims even though they are out of network.
In-network benefits

There are no claims for you to file when you go to a participating vision specialist. Simply pay your copay and, if applicable, any amount over your allowance at the time of service.

In-network value added features:
  • Additional lens enhancements: Average 20-25% savings on all other lens enhancements.
  • Savings on glasses and sunglasses: Get 20% savings on additional pairs of prescription glasses and nonprescription sunglasses, including lens enhancements. At times, other promotional offers may also be available.
  • Laser vision correction: Savings averaging 15% off the regular price or 5% off a promotional offer for laser surgery including PRK, LASIK and Custom LASIK. This offer is only available at MetLife participating locations.
Eligible employees receive basic life insurance from MetLife in amount equal to $100,000

Eligible employees may also choose to elect supplemental insurance coverage for themselves and their spouse and unmarried dependent children. Employees may purchase these plans and benefits through pre-tax payroll deductions:

  • Disability Insurance – Replaces a portion of your income to help make ends meet if you become disabled from a covered accident or covered sickness.
  • Accident Insurance – Helps offset the unexpected medical expenses, such as emergency room fees, deductibles and copayments, that can result from a fracture, dislocation or other covered accidental injury.
  • Cancer Insurance – Helps offset the out-of-pocket medical and indirect, non-medical expenses related to cancer that most medical plans don’t cover. This coverage also provides a benefit for special cancer-screening tests.
All employees may enroll in the DataClarity Corporation 401(k) Retirement Plan managed by Fidelity Investments. Employees may contribute up to 60% of their annual salary to the plan on a pre-tax basis to the IRS annual maximums.

The DataClarity 401(k) Retirement Plan has the following benefits and features:

The DataClarity Corporation Stock Plan allows eligible employees the opportunity to benefit from long-term growth and the financial success of the company through the granting and purchase of DataClarity Corporation Incentive Stock Options. The granting of Incentive Stock Options to eligible employees through periodic stock option grants is based upon salary, responsibility, position and performance, and subject to Board of Directors’ approval and program guidelines.

Vacation/Personal/Sick Days

DataClarity employs a flexible system called Paid Time Off (PTO). PTO is an annual allowance of days that can be utilized for vacation, personal and sick days. During the first five (5) years of employment with DataClarity, eligible full-time employees accrue Paid Time Off (PTO) at the rate of fifteen (15) working days per year. At the beginning of the sixth year of employment, Paid Time Off (PTO) accrues at a rate of twenty (20) working days per year. Once PTO hours accrue on a monthly basis, there is no waiting period before they may be used. PTO hours reset at the beginning of each calendar year (January 1st), and therefore do not carryover from prior periods.


In calendar year 2019, DataClarity Corporation will provide its regular, full-time employees with a total of ten (10) paid holidays including their birthday.
